19 Action News Investigative Reporter Scott Taylor has discovered that Cuyahoga County officials are investigating a social worker who claimed to be visiting a client at their home, when it was discovered that the client had actually died two weeks prior.
The social worker is employed with the county's Division of Senior and Adults Services.
A supervisor was reviewing the client's file after that person's death and discovered the alleged fake visit.
Agency officials tell our Scott Taylor that the social worker appears to have gone home instead of visiting the client.
Officials with the agency are now investigating exactly what happened, but they say it appears the social worker entered a January visit date into the agency's system.
